How They Work: One Receptor Versus Two
Semaglutide mimics GLP-1, a gut hormone that slows gastric emptying, increases satiety signaling in the brain, and helps regulate insulin release. Tirzepatide does the same at the GLP-1 receptor but also activates the GIP receptor, a second incretin pathway. The added GIP activity is the leading mechanistic explanation for tirzepatide's larger average effect size in trials, though exactly how much each receptor contributes is still being studied.
This is a simplification of a genuinely complex signaling system, and mechanism alone doesn't fully predict how any one person will respond. Some patients do better on semaglutide despite the group averages favoring tirzepatide, and vice versa.
FDA Approval, Dosing and the Injection Pen
Both come in prefilled, single-use auto-injector pens, dosed once weekly on the same day each week regardless of meals. The SURMOUNT program is the equivalent series for Zepbound; SURMOUNT-1 tested tirzepatide against placebo over 72 weeks and reported dose-dependent average weight loss reaching roughly 20% at the highest dose.
The more directly useful data point is SURMOUNT-5, an actual head-to-head trial that put tirzepatide against semaglutide 2.4 mg in the same study population rather than comparing across two separate trials. It found tirzepatide produced meaningfully greater average weight loss over 72 weeks. These are group averages across large trial populations — plenty of individuals land above or below the mean in either arm.
Side Effects: Where They Overlap and Where They Differ
Both drugs cause nausea, vomiting, diarrhea, constipation and reduced appetite, concentrated around dose increases. Some patients tolerate one better than the other for reasons that aren't fully understood, and switching between the two for tolerability is something prescribers do in practice. Rarer but more serious risks shared by both include gallbladder problems and pancreatitis, plus a boxed warning about thyroid C-cell tumors seen in rodent studies, of undetermined relevance to humans — a personal or family history of medullary thyroid carcinoma or MEN 2 is generally a reason to avoid both.
Weight Regain, Maintenance and Stopping Either Drug
Both are studied and generally prescribed as long-term, ongoing therapy for a chronic condition rather than a short course. Extension data for both drugs shows weight regain is common after stopping, consistent with obesity being treated pharmacologically the way other chronic conditions are — not something a fixed course of treatment permanently resolves.
