This is consistently reported across weight loss methods — caloric restriction, bariatric surgery and pharmacological weight loss — and it is not a unique property of GLP-1 medications. It happens for understandable reasons: bone adapts to the mechanical load placed on it, and carrying less weight is less load.

Studies examining bone outcomes with GLP-1 receptor agonists have produced mixed findings, with some suggesting the bone effects may be less pronounced than with equivalent weight loss by other means, and others finding reductions consistent with the degree of weight lost. The research is ongoing and the picture is not settled.

Loading Comes First

Bone remodels in response to mechanical strain. Resistance training and impact loading provide that strain directly, which is why they have substantially stronger evidence for bone mineral density than any supplement. Trials of progressive resistance and impact training in postmenopausal women with low bone mass have reported meaningful bone density improvements — results no supplement regimen has matched.

That is the uncomfortable part of this page: the effective intervention requires effort rather than a purchase. Two to three resistance sessions a week, including loaded movements through the hips and spine, is the meaningful dose. Walking helps general health enormously and is a comparatively weak bone stimulus, because the loads are low and the body adapts to them quickly.

Adding external load to walking — a weighted vest, a loaded pack — raises the strain of an activity already in your routine, which is a low-friction way to increase loading. It comes with a real caution: anyone with existing vertebral compression fractures, diagnosed osteoporosis or spinal problems should not add axial load without clinical guidance.

Vitamin D: Test, Then Supplement

Vitamin D is required for calcium absorption, and deficiency is genuinely common. It is also one of the few nutrients where testing before supplementing is straightforward, cheap and genuinely informative, because the right dose depends on where you are starting from.

Supplementation ranges commonly used sit between 800 and 2,000 IU daily for maintenance, with higher repletion doses used under clinical guidance where deficiency is documented. Vitamin D3 is generally preferred over D2 for raising and maintaining blood levels. Because it is fat-soluble, absorption improves when taken with a meal containing some fat — which on appetite-suppressing medication may need a little planning.

Two cautions worth stating. Very high intermittent doses have in some trials been associated with increased rather than reduced falls, so more is not better. Calcium, Protein and the Rest

Calcium is best obtained from food where possible, because dietary calcium comes with other nutrients and does not carry the questions that have been raised about supplemental calcium and cardiovascular outcomes — a debated area rather than a settled one, and a reason not to supplement reflexively.

On a GLP-1 medication, food-first is harder. Reduced intake makes hitting calcium targets through diet more difficult, and dairy is sometimes among the foods people tolerate less well. If a supplement is needed, calcium citrate is absorbed adequately without requiring stomach acid, which makes it the more sensible choice for anyone on acid-suppressing medication — common in this population given reflux.

Protein deserves a mention because it is frequently overlooked in bone discussions. Adequate protein intake supports bone as well as muscle, and protein is precisely what falls when appetite is suppressed. Vitamin K2 is often bundled into bone supplements; the evidence is interesting and considerably less established than for vitamin D and calcium.
