GLP-1 receptor agonists produce the most clinically meaningful weight loss pharmacotherapy has ever seen — but their GI side effects are a real barrier to tolerability, and several secondary effects (hair loss, "Ozempic face," joint pain) have attracted significant media attention without always being accurately characterized. This guide separates the direct pharmacological effects, the indirect effects of rapid weight loss, and the genuinely rare but serious adverse events that require medical attention.
The core mechanism behind most GI side effects is straightforward: GLP-1 receptors are expressed throughout the gastrointestinal tract and brainstem. GLP-1 agonism activates the nucleus tractus solitarius (NTS) in the brainstem — the same region activated by nausea signals — and slows gastric motility. These effects are integral to how GLP-1 agonists produce satiety and lower blood sugar; eliminating them would eliminate the efficacy. The management strategy is therefore not to eliminate the mechanism but to expose the GI system to it gradually — hence the titration schedule.
