The Big Four (Real and Common)
1. Nausea — Real, Most Common, Mostly Temporary
Prevalence: 40-44% of patients in STEP trials reported nausea. It's the most common side effect by a wide margin.
Why it happens: Delayed gastric emptying (food sitting in your stomach longer) triggers the nausea response. The effect is strongest during dose escalation periods.
What the data shows: In STEP 1, nausea was most common in weeks 4-12 during escalation and declined substantially at maintenance dose. By week 68, less than 10% of patients reported ongoing nausea.
How to manage it: Eat smaller meals. Avoid lying down immediately after eating. Avoid high-fat, spicy foods in early weeks. Ginger tea is evidence-supported. If severe, your provider can slow dose escalation — this almost always resolves it.
2. Constipation — Real, Often Underreported
Prevalence: 24-35% of patients in clinical trials. Often underreported because patients don't mention it proactively.
Why it happens: GLP-1 receptors in the enteric nervous system (gut's own nervous system) slow motility throughout the GI tract — not just stomach emptying.
Management: Fiber intake (25-35g/day), hydration (your intake is lower on GLP-1, so hydration requires deliberate attention), and magnesium supplementation (300-400mg glycinate or citrate) are the first-line interventions. MiraLax is safe to use alongside GLP-1 if needed.
3. Muscle Loss — Real, Significant, and Preventable
The data: Studies suggest 25-40% of weight lost on GLP-1 medications may be lean mass (muscle), compared to 20-25% from dietary restriction alone. This is the most clinically important side effect that receives the least attention.
Why it matters: Muscle mass is metabolic currency. Losing significant lean mass slows your resting metabolic rate, weakens you, increases fall risk, and compromises body composition even if the scale number looks good.
The fix — and it's well-established: Protein intake of 1.2-1.6g per kg of body weight per day, combined with resistance training 2-3x per week, dramatically reduces lean mass loss. Studies (including STEP 5 extension) show patients who followed protein + resistance protocols retained 3-4x more lean mass than those who didn't.
4. Injection Site Reactions — Real but Minor
Subcutaneous injections (for weekly semaglutide) cause redness, bruising, or mild pain at the injection site in roughly 10-15% of users. Rotating injection sites (abdomen, thigh, upper arm) and proper technique resolve most cases. Not a reason to discontinue for most patients.
The "Ozempic Face" Question
Media coverage has made "Ozempic face" — facial volume loss and sagging associated with rapid weight loss — a frequently asked-about concern. Here's the honest assessment:
Facial volume loss is a consequence of rapid weight loss generally, not specifically of GLP-1 medications. Any significant and relatively fast weight loss (10%+ of body weight) can produce this effect because facial fat is part of the fat being mobilized. GLP-1 medications don't cause uniquely more facial fat loss than other weight loss methods producing similar results.
Verdict: Real, but it's a "rapid weight loss" effect, not a "GLP-1 specific" effect. Slower dose escalation and the protein/resistance protocol that protects muscle also partially mitigates this by slowing fat loss rate and preserving overall body composition quality.
Overstated Concerns
Pancreatitis — Rare, Not Proven Causal
Early case reports suggested a possible link between GLP-1 agonists and pancreatitis. Subsequent large-scale trials (LEADER, SUSTAIN, STEP) found no statistically significant increased risk versus placebo. The FDA label carries a precautionary warning, but the evidence for causal risk is weak. Patients with personal or family history of pancreatitis should discuss with their physician.
Thyroid Cancer — Animal Data Only
Rat studies showed increased C-cell thyroid tumors with high-dose GLP-1 agonists. Human data from trials with tens of thousands of patients has shown no increased thyroid cancer incidence. GLP-1 receptors in human thyroid tissue are expressed at much lower levels than in rats. The warning exists on the label; the human evidence doesn't support elevated concern for most patients.
Cardiovascular Risk — Opposite Is True
GLP-1 agonists reduce major cardiovascular events by 20% in high-risk patients (SELECT trial, 2023). They reduce blood pressure and triglycerides. The cardiovascular signal is strongly positive, not negative.
When to Contact Your Provider
Seek medical attention if you experience:
- Severe or persistent abdominal pain (especially radiating to back — potential pancreatitis)
- Nausea/vomiting that prevents you from eating or drinking for more than 24 hours
- Vision changes (rare but reported with semaglutide)
- Symptoms of low blood sugar (if also on insulin or sulfonylureas)
- Lump or swelling in neck, difficulty swallowing (thyroid)
