The most common side effect, peaking during dose escalation and typically resolving within 4–8 weeks at a stable dose. Mechanism: GLP-1 receptors in the brain's area postrema (the nausea center) are directly activated. It's not stomach damage — it's a central nervous system signal.
When it's worst: The first 1–2 weeks at each new dose level. This is expected, not a sign something is wrong.

No human clinical data confirms increased thyroid cancer risk. However, semaglutide carries a black box warning and is contraindicated in people with personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
GLP-1 medications slow gastric emptying — their intended mechanism. People with pre-existing gastroparesis can experience significant worsening. This is a contraindication in most prescribing guidelines. Relevant to diabetic patients who may have autonomic neuropathy affecting the stomach.
Not a medical emergency, but worth knowing about. Rapid weight loss redistributes fat loss from body areas including the face, causing a gaunt, aged appearance. This is not semaglutide-specific — it occurs with any rapid significant weight loss. Higher protein intake and slower weight loss pace help preserve facial volume.
The STEP 1 NEJM data showed that approximately 39% of weight lost on semaglutide was lean mass — a ratio worse than diet-alone approaches. This is not a GI side effect — it's a metabolic consequence of caloric restriction without adequate protein and resistance training stimulus.
